However, the irreversible loss of a large part of the stomach is a disadvantage.

Weight reduction can result in a significant improvement in the general state of health, as many secondary diseases are also favorably influenced.

The gastric sleeve is a strictly restrictive procedure in which a large part of the stomach is removed.

 

This means that food can only be consumed in small portions. 

 

About 30% of patients lose 60% of their excess weight after gastric sleeve surgery and their accompanying illnesses are largely eliminated. Approximately 10% of patients do not achieve this reduction in weight, but lose enough weight that they are satisfied with the result, since most of the comorbidities have been eliminated or significantly improved. The rest of them experience minimal weight loss and are not satisfied with the result.

The causes of insufficient weight loss are:

No change in eating habits, inadequate physical activity, and continued behavioral errors related to food intake.

There are seldom anatomical reasons for the lack of weight loss. In some cases, too little was removed from the stomach during surgery. Most of the time, however, patients have consumed excessively large amounts of food per serving and thus gradually caused an increasing widening of the stomach tube (dilation). The stomach tube is a muscle that wears out with constant overstretching, thereby becoming larger again and also allowing larger portions.

A scarred narrowing in the stomach tube (stenosis) usually leads to more frequent vomiting and heartburn. This condition, if it occurs early, can possibly be treated with an enlargement. This is done without surgery with the gastric mirror through which a balloon is inserted to expand it. Another possibility is to insert a self-expanding stent (a tube that widens the constriction) if the constriction occurs prematurely. This also happens with the stomach mirror. The stent is removed again after 4-6 weeks. If these measures do not bring about permanent improvement, an operative correction must be carried out. The tube stomach is then preferably converted into a bypass.

 

Many patients suffer from heartburn even before the operation. Heartburn is caused by gastric juice flowing back into the esophagus. In many patients, these symptoms improve when their body weight drops. In some patients, however, there is no improvement or even an increase in heartburn. In about 8% of the patients who had no heartburn before the gastric sleeve surgery, these symptoms develop again. Most heartburn can be improved or eliminated with acid-blocking medication. However, when gastric juice runs through the esophagus to the larynx, especially when lying down, coughing fits occur, which can be very painful and cannot be eliminated with tablets. In this case, only a corrective operation remains, which usually also consists of converting the gastric tube into a bypass.

In the case of insufficient weight loss and a highly dilated gastric sleeve, further reduction in size of the gastric sleeve rarely leads to good results. This measure should then be combined with an additional bypass operation. This means that the sleeve stomach is made smaller and the small intestine is also partially eliminated in front of or behind the gastric outlet.

Information on corrective operations after a bariatric procedure

If you have had weight loss surgery before, you may experience some difficulty months or years after the surgery. This is rather rare and should be discussed in practice. At this point, we would like to provide you with some information in advance so that you can describe and discuss your problems in more detail during the medical consultation. 

 

The following groups of problems can be identified regardless of which procedure was previously carried out on you:

 

insufficient weight loss

considerable increase in body weight despite previous good weight loss.

Symptoms from frequent vomiting, very frequent heartburn, rising gastric juice with coughing fits, especially when lying down, constrictions in the upper digestive tract with obstruction of passage, ulcer formation in the small gastric pouch, insufficient weight loss or a significant increase in body weight after the operation are actually never caused by changes in the operated stomach . Most of the reasons are rather failure to observe the dietary recommendations, insufficient physical activity (going swimming, walking the dog, climbing two flights of stairs a day is not enough). Behavioral patterns that led to overweight and obesity are often retained despite intensive advice or resumed in situations of frustration. These conditions require extensive further clarification and usually also psychotherapy in order to work through and eliminate the undesirable development.

Anatomical reasons in the operated stomach or small intestine are very rarely the cause of the above-mentioned weight changes. A defective gastric band can lead to an increase in weight, a fistula between the small stomach pouch and the stomach that is switched off, or a sleeve stomach that widened over time.

In order to find out such causes, an X-ray examination and a gastroscopy are required in advance.

Typical complications of the individual surgical procedures are presented below:

 

Gastric band:

A gastric band that is as tight as possible leads to frequent vomiting several times a day. As a result, the stomach located below the gastric band can turn up through the gastric band and thus lead to an obstruction of passage. This condition is called slippage. The complete emptying of the gastric band immediately eliminates the discomfort. As a rule, however, the slippage cannot be removed without surgery.

An extremely tight gastric band also leads to a widening of the esophagus and often to a long-term lingering of food in the esophagus with corresponding inflammatory reactions. When lying down, gastric juice and food particles can run back into the trachea. This causes coughing fits.

In rare cases, the gastric band can also migrate through the stomach wall and then lie in the stomach, causing weight gain again (erosion).

All of these conditions require correction. This consists of removing the gastric band. The mere removal of the gastric band leads to an increase in body weight in 99% of cases. It should therefore be considered to combine the removal of the gastric band with another procedure (bypass, gastric sleeve).

It is known from great statistics that reinsertion of a gastric band does not give good results.

Gastric bypass:

For gastric bypass with regard to insufficient weight loss or renewed weight gain, the same statements apply as just given for the sleeve stomach. The small part of the stomach (stomach pouch, technical term: stomach pouch) can also expand if the food portions are too large and absorb more food again. What is more important, however, is that the connection between the stomach pouch and the subsequent small intestine also expands and thus the food can slide out of the pouch into the small intestine much faster. This condition is basically due to an undisciplined eating behavior. A correction in the sense that the pouch and the connection between the pouch and the small intestine are made smaller rarely leads to good results, the weight loss is small and does not last long. Studies are currently underway to test whether an additional narrowing of the stomach pouch with a plastic ring results in an improvement. As a rule, however, renewed nutritional counseling and psychotherapy are required in order to treat the persistent food intake disorder.

Constrictions (stenosis) between the stomach pouch and the subsequent small intestine are rather rare, usually occur a few weeks after the operation and can usually always be removed with a balloon widening with the help of the gastric level.

About 7% of patients with gastric bypass develop ulcers at the transition from the gastric pouch to the small intestine. These ulcers can be observed almost exclusively in heavy smokers or in patients who continue to take painkillers from the group of rheumatoid drugs. The ulcers can usually be healed with medication. It is important, however, that the patient gives up smoking and switches pain relievers to stomach-friendly drugs. In extremely rare cases (bleeding, gastric wall perforation), surgery is indicated for correction.

 

Summary:

Every operation has its own specific risk. The first interventions for weight loss show a pleasingly low rate of complications despite the mostly considerable concomitant illnesses. This does not apply to new interventions. Even with a reduced weight, the complication rates are considerably higher. If complications between 5-7% occur during the first operation, the complication rate for the first corrective operation is 15-25%, for further corrective operations the risk of complications rises to 50%. Correcting an existing procedure (e.g. correcting a gastric bypass) does not produce good results; as a rule, a different method should be used. In our own experience, we then see patients who benefit very well from the new procedure. But there are also patients who, despite the changed surgical procedure, are not very successful.

In this respect, before each new intervention, you should think very carefully about what success you can hope for with which method and what risk you want to take for it.

Even in the long term, however, some patients complain of a constant feeling of pressure or fullness, belching and heartburn. Conventional gastric balloons are endoscopically inserted through a tube into the patient's stomach by gastroscopy. Allurion has developed a new type of gastric balloon treatment: the Elipse ™ swallowing balloon. Elipse ™ Gastric Balloon Treatment: How It Works

The Elipse ™ gastric balloon is placed in the patient's stomach and reduces its volume. As a result, patients can no longer eat as much and become full more quickly. The gastric balloon can thus help people lose weight who have not had any success with other methods and help them lead a healthier life.

What is special about the Elipse ™ gastric balloon is the type of treatment, i.e. the insertion and removal of the balloon. The Elipse ™ gastric balloon is a so-called swallowing balloon. It is empty by the patient, swallowed in the form of a capsule and, as soon as it has reached the stomach, filled with a liquid via a fine tube attached to the capsule. Gastric pacemaker

Electrical currents in the stomach are responsible, among other things, for feeling full and for emptying the stomach. The gastric currents are generated by means of a pacemaker, similar to a cardiac pacemaker
